I give my permission for my youth to participate in a Field Trip. I understand that although the youth will be supervised by staff and volunteers, I do assume the risk of my youth’s

participation in the event.

I certify that my youth is physically and emotionally able to participate in the activities planned for the trip. I understand that I am responsible for informing staff members of any medical conditions, allergies, or other special needs that my child may have. I authorize staff members to seek medical attention for my child if necessary and agree to assume responsibility for any medical expenses incurred.

I acknowledge that I will not seek to have the Armory Youth Project held liable if any accident, injury, loss of property or other circumstance or incident occurs during or because of my youth’s participation in the field trip. This release of liability includes accident, injury, loss, or damages to the youth, as well as to other individuals or property which may result from the youth’s participation in the event. I hereby release and agree to hold harmless the Armory Youth Project, its officials, agents, and employees from any claims arising out of my youth’s participation in the event.
