Authorization and Release
I hereby authorize the Armory Youth Project and/or its agents to conduct a ICHAT comprehensive background check ,DHS (Dept of Homeland Security) or other consumer reporting agency, which may include, but is not limited to: criminal history, child abuse registry, sex offender registry, driving records, and verification of identity and previous employment/volunteer history.
I understand that the purpose of this background check is to ensure the safety and well-being of the youth and staff involved with the Armory Youth Project. I understand that all information obtained will be held in strict confidence and used solely for the purposes of evaluating my suitability for volunteering/employment.
I release and discharge the Armory Youth Project and any individuals or entities obtaining or providing information pursuant to this authorization from any and all liability for any damages that may arise from the use of such information.
This authorization shall remain on file and shall serve as ongoing authorization for the Armory Youth Project to procure background reports at any time during my involvement with the organization.
