Please fill out this form to purchase an Activity Pass good towards AYP $5 community classes/activities
The Activity Pass will have boxes worth $5 each when pass is used, the date will be put in one box (or two if going to two exercise classes)
Purchasing an Activity Pass Card saves you money.
Purchasing multiple cards? Please include first and last name that should be put on the Pass Cards.
Purchasing as a gift to be sent directly to recipient? Please fill out the additional information needed to be put on the note that will be sent with the Pass Card, exactly how you want it worded.
