Participation at Armory Youth Project’s facilities and in AYP Programs and Events is subject to the observance of AYP expectations, rules and procedures. Any student, volunteer or visitor who violates this code is subject to discipline, up to and including removal from regular programming.

At Armory Youth Project: The mental, emotional and physical safety and well-being of our youth is our main priority.

Parents are responsible for their student at all times. Parents must coordinate pick up and drop off. Dial A Ride is available at no cost to the student with prior arrangements within their service hours.  Students must be picked up by AYP closing time. See hours for details. 
Parents may coordinate pick-up restrictions with AYP and student daily as needed. Students are required to check in and check out daily. AYP is a single-entry program. Once signed out students are not allowed back in without prior approval.  AYP does not have the resources to physically prevent youth from leaving. Parents must set expectations with student. Parents will be contacted with any concerns. AYP is a student drop-in program. Students are taught independence and choice. Students choose what programs to participate in. Students are requried to to sign-in when they arrive and sign-out when they leave for the day. 
AYP is not responsible for students once they have signed out of AYP.  

Personal Belongings

Youth are responsible for personal items. Personal cubbies are provided with camera surveillance. AYP is not responsible for any lost or stolen items and will not replace them.

I acknowledge that participation in activities, events, or programs involves risk to the participant (and to the participant's parents or guardians, if the participant is a minor), and may result
in various types of injuries including, but not limited to, the following: sickness, exposure to infectious/communicable disease, bodily injury, death, emotional injury, personal injury, property damage, and financial damage.

In consideration of the opportunity to participate in AYP activities, events, or programs, the participant (or parent/guardian) acknowledges and accepts the risks of injury associated with participation. The participant (or parent/guardian) accepts personal financial responsibility for any injury or other loss sustained during AYP participation, as well as for any medical treatment rendered to the participant that is authorized by the Armory Youth Project or its agents, employees, volunteers, or any other representatives (collectively referred to as the "activity sponsor"). Further, the participant (or parent/guardian) releases and promises to indemnify, defend and hold harmless the activity sponsor for any injury arising directly or indirectly outside the described activity, whether such injury arises out of the negligence of the activity sponsor, the participant or otherwise.

If a dispute over this agreement or any claim for damages arises, the participant (or parent/guardian) agrees to resolve the matter through a mutually acceptable alternative dispute resolution process. If the participant (or parent/guardian) and the activity sponsor cannot agree upon such a process, the dispute will be submitted to a three-member arbitration panel for resolution in accordance with the rules of the American Arbitration Association.

Photo Release:
If you participate in the Armory Youth Project programs and events there is a possibility you will be photographed and/or filmed. Your picture and or video may be used for promotional purposes.

AYP is funded through donations, sponsorships, events, and grants. AYP receives partial funding from the State Of Michigan to serve your child. Michigan State University and Public Policy Assoiciates are contracted to evaluate program quality and impacts. By enrolling my child in this program, I agree that AYP will share my attendance information and demographic information with the contracted evaluators. All information will be kept condidential and used soley to evaluate the program.

By signing below, I acknowledge that I have read, understood and agree to the above statements.

* I have received a copy  and agree to the AYP code conduct expectations.
*I agree that enrollment in the AYP program is voluntary. 
*I agree to contact AYP if my contact information changes.
*I give permission to my child to sign up for any AYP program including field trips. 
* I understand that all programs information is available to me on the website and in the monthly email blast. 
* I agree to communicate with child any program concerns and contact AYP with any restrictions.
* I agree that if my child needs medication during the program that I will communicate with AYP staff and my child of proper use. I futher agree to sign a medication authorization form and bring the medication in its orginal prescription bottle.
*I agree to give permission to AYP staff and volunteers to apply insect repellent, sunscreen, and antibacterial cleanser to my child's skin when needed. I understand that I can communicate to my child and AYP staff with any concerns. 
* I agree to give permission in the case of an emergency to AYP staff and volunteers to call for emergency medical treatment for my child.
